This Vaccine is FREE 

and will be administered at 

Faith Fellowship Church in San Leandro, CA and is open for Alameda County Residents.

*Because of the shortage of Johnson & Johnson we are unsure if this will be Johnson & Johnson (1-shot) or 

Pfizer (2-shots, 3 weeks apart).  If we offer the Pfizer, we will have another day for everyone to return 3-weeks later.

This is NOT a drive-through.  You will need to register and come inside the building.

You MUST Register to be able to receive the vaccine.  
*A valid photo ID with your birthdate is required.

• You LIVE in Alameda County.  We require a valid Driver's License or ID that shows your address or proof of Alameda County address.
• You are over 18 years old.
